Creekside Received Third Consecutive Deficiency-Free Rating from State Survey

At Creekside Living in Grundy Center, IA we’re proud to uphold a deep commitment to the health, safety, and well-being of those who have entrusted us with their care. This commitment begins with strict adherence to regulations established by the Centers for Medicare and Medicaid Services and the State of Iowa — and we’re excited to share some great news!

Creekside Living has once again received a Deficiency-Free Rating from the Iowa Department of Inspections and Appeals during our annual survey. This marks our third consecutive deficiency-free result — an achievement earned by only a small percentage of senior living communities across the state.

The state survey process is rigorous, covering more than 500 quality, safety, and performance metrics, including medication management, care planning, residents' rights, nutrition services, and more. During the multi-day inspection, state officials toured our Creekside campus, reviewed clinical records, and conducted interviews with residents and families to ensure that we meet or exceed every regulatory requirement.

A deficiency-free rating means that Creekside Living has met every standard of care and safety without exception.
